[ExandraTM logo appears] [Narrator] Introducing ExandraTM
A new online tool developed to assist health care practitioners
in creating customized treatment plans for people living with Type 2 diabetes
This clinical support tool can help inform treatment decisions by
converting complex guidelines into clear, personalized, actionable patient recommendations.
It’s simple and easy to use.
Even though Type 2 diabetes is one of the leading causes of premature cardiovascular death,
and significantly contributes to cardiovascular morbidity, less than 20% of people living
with Type 2 Diabetes and cardiovascular disease receive all of the guideline
recommended therapies.
[ExandraTM logo appears] Exandra helps to address these challenges by
providing 3 key benefits.
1. Supports decision-making.
[Joanne Lewis, Executive Director, HCP Education and Engagement. Diabetes Canada] [Joanne] Working together with the patient
and using the clinical practice guideline recommendations to come up with a
treatment plan that will work for the individual is really important.
It helps to have a consistent message, so that when a person talks to their pharmacist,
or their family doctor or perhaps their diabetes educator, their hearing the same message
and that same message is coming from those guidelines.
[Narrator] Exandra supports guideline implementation by offering individualized
patient-specific recommendations. This personalized approach is
essential for people living with Type 2 Diabetes.
2. Empowers patients.
Exandra presents a visual output of the patient's treatment plan
which helps them engage with their own disease management.
[Dr. Jeffrey Habert, Family Physician and Member of The Dissemination & Implementation Committee of Diabetes Canada] [Dr. Habert] I personally treat about 300 patients with Type 2 Diabetes.
When they understand the benefits of their treatment plan, their much more likely to
embrace and adhere to it.
[Narrator] 3. Increases transparency and collaboration.
Exandra provides a printable, saveable and shareable treatment plan.
It streamlines communication and provides transparency across multi-stakeholder care teams.
[Dr. Habert] If everyone on the team can see the same Diabetes Canada recommendations,
there is less chance of miscommunication between practitioners. For example
between family physician, nutritionists, diabetic educators and specialists.
I would strongly recommend Exandra to other health care practitioners treating patients with
Type 2 Diabetes. Exandra is a valuable tool to help health care practitioners optimize the
care of patients with Type 2 Diabetes.
